Compliance

We are a Medicare-certified, Joint Commission accredited home health agency, licensed by the State of Florida. Our compliance program is how we make sure the care we bill for is the care we deliver — and that everyone who works for us knows the standard.

Our compliance program

A&Z Health Services, Inc. maintains a compliance program covering how we hire, train, document, and bill. It applies to every person who represents us in a patient's home, whether they are on our payroll or working with us under contract.

Code of conduct

Written standards of ethical conduct that apply to everyone who works for us — office staff, clinicians, aides, and contracted providers alike.

Regulatory compliance

We operate under the federal Conditions of Participation for home health agencies (42 CFR Part 484) and Florida's home health licensure law and rules (Ch. 400 Part III and Ch. 408 Part II, Florida Statutes; Rule 59A-8, Florida Administrative Code).

Fraud, waste & abuse

Policies and monitoring built around the federal False Claims Act, the Anti-Kickback Statute, the Stark Law, and Florida's Medicaid fraud statutes — covering documentation, billing, and referral relationships.

HIPAA & privacy

Protected health information is safeguarded under HIPAA and Florida privacy law, with access limited to the people involved in your care. Our full Notice of Privacy Practices explains your rights.

Training

Compliance training is required of every employee and contractor at hire and annually thereafter, and is documented.

Oversight

Our Administrator serves as the compliance contact and is responsible for the program, for reviewing reported concerns, and for corrective action.

Reports are handled confidentially and reviewed by our Administrator. You may report anonymously if you prefer — though leaving a way to reach you helps us investigate properly.

Reporting outside the agency

You always have the right to raise a concern directly with a regulator instead of, or in addition to, telling us. Nothing on this page limits that right.

  • Florida Agency for Health Care Administration (AHCA) — complaints about a licensed home health agency. Our licence number is #299995051. File a complaint online or call 1-888-419-3456.
  • The Joint Commission — quality and safety concerns about an accredited organization, reviewed by their Office of Quality and Patient Safety. Report a patient safety concern or call 1-800-994-6610. They ask that reports come through the online form or by phone.
  • U.S. Department of Health & Human Services, Office for Civil Rights — HIPAA privacy complaints and discrimination complaints under Section 1557. File with OCR or call 1-800-368-1019 (TDD 1-800-537-7697).
  • HHS Office of Inspector General — suspected Medicare or Medicaid fraud, waste, or abuse. Report to the OIG hotline or call 1-800-HHS-TIPS.
